Sending Us a Sample: it's as Easy as 1,2,3:
Step 1: Print and complete this diagnostic form and mail it with your turf sample.

Step 2: Collect 1 or 2 symptomatic cup cutter samples. Then, wrap them tightly in aluminum foil, and pack them securely in a box.
Step 3: Ship your samples overnight (Signature NOT Required) to:

Once a sample is received, you will be sent a confirmation email.
